The film is set in a nearby town immediately after the events of the first film. The monsters have made it into a small neighboring town in the middle of nowhere and the locals have to band with the survivors of the bar' slaughter to figure out how to survive.
The film opens with brief clips from the previous installment including the survivors driving off in the brown Trans Am into the sunset. As the movie begins, the Biker Queen spots a dog running around with a part of an arm, she promptly shoots the dog several times, and takes the hand, matching it to her own finger it completes the word 'Sister' a mutual tattoo. Upon discovering the survivor Bartender from the previous film, she tortures him into telling her who killed the biker from the previous film, who he reveals is Bozo and points to a town where he lived and would have been hanging out. She knocks the old man out and takes him with her down the road to the town.
Meanwhile, the town becomes overrun with the creatures, starting with a pair of small Mexican wrestlers. While one is having sex with his girlfriend she is attacked and pulled through the window, being decapitated at the same time, the pair scream and run out of the house. A bloody slaughter at the jailhouse shortly follows, where a man in the cell for dealing Meth witnesses and promptly takes the keys off of one of the guards. A montage clip of him says that he "Will die soon, and horribly" the scene changes. Next up is the Slasher, whose name is derived from his Auto Sales reputation of 'slashing' the high prices of his cars to low and affordable prices. He makes a sale and as the parents and baby leave, he is spooked by an attack from one of the creatures. He escapes in his car, only to discover his wife (named Secrets) and his employee (Greg) had been having sex in the back of the car. He promptly fires Greg for this just before running into one of the creatures.
The following morning, the Biker Queen and three of her biker friends arrive in the deserted town, seemingly ignoring the dead bodies and decapitated heads in the streets as they look for Bozo. They cross paths with Slasher, Secrets and Greg and one of the Biker Girls is killed as the wall of their shelter collapses, the group escapes into the neighborhood and split ways. Biker Queen, Bartender, Tat Girl and another Biker Girl make their way to Bozo's apartment where they encounter three people across the hall still alive and trying to make the newcomers leave their hiding place. A firefight ensues and the civilians are slaughtered by the bikers. One girl comes out saying she's not armed, but when she crosses eyes with the Bartender he recognizes her as Honey Pie, the girl who left them at the bar. He brutally attacks her, biting a piece of her ear off to the amusement of the bikers. She draws a gun, which Biker Queen recognizes as her dead biker's with the words "Harley Mom" on it. When she attempts to take the gun back, the Bartender strikes again, knocking Honey Pie out the window and onto the street. She then flees.
Slasher, Greg and Secrets are called by the wrestlers and just before they find them, they are ambushed by the bikers. The group makes their way to the brothers' garage which prompts for their montage introduction as "Thunder and Lightning" Thunder brings the noise, Lightning brings the pain. As well as their grandmother, an elderly and properly appearing Spanish woman. As the day wears into night, the group comes up with a plan to use the brothers' key making skills to make the key for the Jailhouse, the safest place in the town, which had ironically been sealed off by the junkie from before. And Honey Pie makes her way to safety by crawling into the vent shaft of a convenience store and falling in, realizing how unsafe it was she finds herself locked in the store and unable to leave through the way she came in.
As the brothers fashion the key in their workshop across the way, Greg begins evaluating a dead creature by disecting it and testing its organs. After accidentally dousing Grandma with scores of a green spewing liquid he ends up inadvertently setting off a spout of foul smelling flatulence, dousing Secrets in its excrement, and showering the women with its semen before they convince him to stop. An eye opens up within its torso, and after examining them it lets out an ear-piercing wail, which Biker Queen stops via shotgun. The screaming had attracted unwanted attention after the brothers return with the key, and they soon have to find their way to the roof. Now trapped, they must contemplate a method of escaping. Honey Pie witnesses the unwilling rape of a cat by one of the creatures and its spawn attacks her in the store. She kills it and finds out using its claw she can slowly chip away at the store's door to get out.
Morning dawns, and with an attack from one of the creatures, Biker Girl 2 is injured, and Slasher decides that to get what they need to get to the jailhouse they need live bait. They hurl the girl off the roof who ends up fleeing from the creatures as they chase after her. Using the distraction (instead of making a run for the jailhouse) the group gets Biker Queen's motorcycle and jury rigs it as a catapault. While making the machine, however they hear a baby crying. The family that Slasher had sold the station wagon to had been attacked and the parents killed. Greg volunteers to save the baby, and during which is attacked by the creatures. He runs but finds he cannot outrun them and throws the baby into the air, letting it fall to the ground where it is promptly eaten. He returns to the roof to the sneers of 'Baby killer' from Thunder. Grandma, who had been doused in the eroding fluids from the dead alien is now little more than a whimpering, whining skeleton. After a brief argument between Lightning and Thunder upon agreeing to test Grandma on the catapault, and a quick confrontation below from a Beer Guy who had been given the same fate, they agree to test her. After a sorrowful goodbye, they launch Grandma successfully to the jailhouse roof. But as expected the impact sending up green blood and body parts kills her. Thunder gets onto the catapault and is launched, however not as successful as the first one, he falls and lands into the middle of the road and is quickly attacked by the creatures.
During the spectacle, Greg notices that the motorcycle is malfunctioning, and before he can request it to be stopped, a piston dislodges from it and rams a pipe up into his jaw and out through the hole in his head. Slasher observes this with a silent, gleeful grin before anyone notices and everyone does notice this after witnessing Thunder's death. Meanwhile, Lightning crosses the street under the disguise of a trashcan to pick up the key that his brother dropped, swearing vengeance as he does so. Thunder crawls away with half of his body gone. His actual death was not witnessed. Thunder reaches the jailhouse and as he opens the door, the dealer inside throws a stick of dynamite out, prompting Lightning to duck down in a meager attempt to protect himself from the explosion. The trashcan launches across the street and the upper ring of it goes flying. Honey Pie, who had managed to barely escape the convenience store makes her way to the railroad tracks laughs at her luck as the debris from the explosion promptly hits her and throws her to the ground, presumably killing her. The others, disturbed at their horrific failure turn to face the monsters who are now breaking into the rooftop. As they start shooting the credits roll, showing over Honey Pie's dead face and a rising pool of blood. After the preliminary credits she is found to not be dead after all and gets up, screaming in anguish and annoyance as she makes her way out of town. The Screen Goes Black.
